Can not add a new volunteer with incomplete data collector information

Describe the bug
All "Add a Volunteer" data fields are mandatory. All fields should not be a requirement (in current design, particularly geographic information).

Some data fields (right now), such as administrative boundaries are very difficult to remember as text fields (e.g. regions, districts, village). Implies that when trying to rapidly add a volunteer, I am adding bogus information as to add the person quickly.

To Reproduce
Steps to reproduce the behavior:

1. Go to https://dev.cbsrc.org/reporting/datacollectors/register
2. Add bare minimum fields (Name, Phone number, Village)
3. Scroll down to "Submit", field shows a red circle with a line through it. Can not hit submit with all fields filled in.
4. See error

Expected behavior
Expect to add Volunteers with minimum information, with the ability to edit and add more at a later time (if/when needed).
