Tidy up queue selection logic

Clarify what the distinction between queues and queue families, add more description of the common setups of queues (how to think about the presentation queue, how its the same as graphics queue 99% of the time), and tidy up the queue selection logic

Reason: Very common confusion in new users, not actually that complicated but appears much more complicated.
