[bug] Scroll back to top when there are errors when trying to submit a new app

Currently when you fill in the data to add a new app and submit the response, if there are any form errors, the errors are displayed on the top of the form, but the page doesn't scroll up and so the error is not easily visible. 

The form should scroll back to top after an error occurs when submitting the form.
