Add support for outgoing calls in the call hierarchy view

This feature request aims at complementing PR #513 (support for call hierarchy view with incoming calls) by adding support for outgoing calls.

For example, clangd (vers. 20) now supports outgoing calls and could be used in CDT LSP.
