Improve handling of loss of carrier on CM4/CM5 #159
Description
When carrier is lost on the CM4/CM5, it looks like in some situations the PHC can be completely trashed.
At the moment, we stop sending timestamps when we lose carrier, and restart when it is regained, but this isn't enough to handle things optimally. We need to reset the combiner so that it starts fresh.
