1. Which part do you want to customize? I mean the name can be customized by simply typing in the text box, but I guess this isn't what you mean.
2. I think this is very similar to How to get each element's own label name from the request data durging form creating in form builder or form the submitted form data? #141 — so the answer is that you need to create a custom solution for that (but if you need help with the implementation, let me know)
