vhpi_printf prefix part NOT aligned

Depending of the internal state, time of the simulation the prefix part of a vhpi_printf is different:

If you want control over the formatting you'll need to format it yourself (i.e. using printf directly). The vhpi_get_time function will give you the current time (in fs) and the current delta cycle.
